John (not his real name) is a client who had too many medications to count. John had been in the hospital 3 times in a two-month period just before I met him. He was confused about what to take and how often to take it. He had three pharmacies and four doctors trying to help him, but none of them were working together.

We installed a monitored medication dispenser and provided a skilled nurse to fill it on a weekly basis. At first it was not perfect. Figuring out what medications were current and making sure each doctor approved of the list was not easy. John actually ended up back in the hospital after two weeks, because he was feeling horrible from taking his prescribed medications properly.

John has diabetes, emphysema, high blood pressure, poor blood flow in his legs, low kidney function, and congestive heart failure. This combination can now account for his 25 prescriptions that you can count in the picture, but he has one Pharmacy and one Geriatric Physician managing his care and that is good. He has had a hospitalization or two over the course of the last 3 years, but overall his health is under control and with our support he is living at home.
